Acne isn’t just a problem for teens. According to studies, those pesky zits and pimples can pop up at any age. To get the skinny on acne, we talked to board-certified dermatologist Cynthia Bailey, M.D., president and CEO of Advanced Skin Care and Dermatology Physicians, Inc. and founder of DrBaileySkinCare.com. Here, she shares her best tips on how to choose the right acne cleanser for you, as well as how to use it to get the best results.

Oftentimes, our first instinct when we encounter bumps, pimples and other blemishes on our skin is to touch it. But if you’ve been dealing with acne for a while, you’ve probably learned by now that these practices only make acne and skin problems worse. For starters, our hands come into contact with more bacteria, pathogens and contaminants than any other part of our bodies. Even when we try to wash our hands often, there is only so much of that we can avoid putting on our faces every time we reach out to touch it. So, there’s the fact that we could be adding harmful bacteria our facial skin to begin with. But it doesn’t end there.
Genetics can also affect how your immune system works. When confronted with bacteria, your skin might erupt in painful red lumps called pustules while someone else might just get a blackhead. Or maybe your friend has sensitive skin that breaks out more often than yours does. Your family history has a lot to do with the type of skin you have and how it looks and feels.
Azelaic acid is a natural acid that is derived from various grains like wheat and barley. It helps reduce acne by preventing skin cell buildup, killing p. acnes bacteria, and reducing inflammation4. This means it can help reduce all kinds of acne, from blackheads to pimples. Blackheads and whiteheads form when dead skin cells combine with sebum, the oil our skin naturally produces and get clogged in a pore. To get rid of this kind of acne, it helps to regulate your skin cell production and exfoliate your skin to remove excess dead skin cells. Azelaic acid is a great way to do both of these things, while also decreasing inflammation and killing bacteria. When the skin is inflamed, it swells slightly, causing the pores to constrict. This traps dead skin cells and sebum inside, leading to blackheads and whiteheads, and if bacteria get trapped as well, then pimples can also form.
Oral antibiotics are recommended for no longer than three months as antibiotic courses exceeding this duration are associated with the development of antibiotic resistance and show no clear benefit over shorter courses. Furthermore, if long-term oral antibiotics beyond three months are thought to be necessary, it is recommended that benzoyl peroxide and/or a retinoid be used at the same time to limit the risk of P. acnes developing antibiotic resistance.
Antibiotics are frequently applied to the skin or taken orally to treat acne and are thought to work due to their antimicrobial activity against P. acnes and their ability to reduce inflammation. With the widespread use of antibiotics for acne and an increased frequency of antibiotic-resistant P. acnes worldwide, antibiotics are becoming less effective, especially macrolide antibiotics such as topical erythromycin. Commonly used antibiotics, either applied to the skin or taken orally, include clindamycin, erythromycin, metronidazole, sulfacetamide, and tetracyclines such as doxycycline and minocycline. When antibiotics are applied to the skin, they are typically used for mild to moderately severe acne. Antibiotics taken orally are generally considered to be more effective than topical antibiotics, and produce faster resolution of inflammatory acne lesions than topical applications. Topical and oral antibiotics are not recommended for use together.
